MUD reports receiving a locate request via the Nebraska One Call Center for a locate of gas and water utilities for a contractor that was working in the area of 11th and Howard Streets.
Monday, Jan. 4
MUD reports completing a request to refresh the locate markings of all MUD facilities, including mains and service, lines in that area
Saturday, Jan. 9
2:51 p.m. - Firefighters are dispatched to a possible explosion at 422 S 11th St., according to the Omaha Fire Department.

2:53 p.m. - MUD reports notifying technicians of a fire with a possible building explosion with natural gas involved, about a minute after hearing it on a radio scanner.
3:04 p.m. - Working fire declared.
3:16 p.m. - MUD says technicians arrived on scene to shut down the gas main and services
3:30 p.m. - Additional MUD technicians begin checking atmosphere levels in nearby buildings, underground vaults and sewers, the utility company says. A gas leak survey of the surrounding area found no leaks.
3:43 p.m. - Second alarm declared as crews fight the fire from the outside until the natural gas is turned off.
4:28 p.m. - MUD says gas to the building was turned off and verified by the Omaha Fire Department.
6:24 p.m. - One person taken to the hospital with a broken leg after slipping on the ice.
7:26 p.m. - Third alarm declared because of the extreme weather.
10:30 p.m. - Roof of the building collapsed, according to the Omaha Fire Department, following collapse of parts of the first floor.
Midnight - Fire declared under control.
Sunday, Jan. 10
2 a.m. - A firefighter broke a hand after falling on the ice, according to Fire Chief Bernie Kanger.
