Berlowitz has been charged in Sarpy County Court with third degree assault, trespassing, and disturbing the peace.

Smith described the altercation to KMTV, saying Berlowitz was loud, agitated, and wouldn't leave when staff asked. "He said he was here on city and personal business," Smith said. "He said, 'You might want to shut that door.' I said, 'No, I'm keeping it open.'

Then he yelled, 'I'm telling you, you might want to shut that door!'"

Smith said Berlowitz's wife had been employed at the dental office until recently.

An arraignment is scheduled in County Court on July 27.
