OMAHA, Neb. (KMTV) — Creighton University Campus Pharmacy reports a data loss of electronic patient records occurred on Tuesday, June 18 at its 2412 Cuming St. location.

The loss affects prescription and refill history, insurance information for all customers. However, no patient records were stolen as this was a data loss and not a breach.

The university says the database needs to be rebuilt, requiring all patient data to be re-entered and new prescriptions obtained from physicians.

If you receive your prescriptions from the Creighton University Pharmacy, you can submit a demographic form securely online.

You can also fax the form at 402-449-4531.

You will need to contact your physician to get new prescriptions written or called in to the pharmacy for your current medications.

The pharmacy apologizes for the inconvenience and is taking all necessary steps to prevent future problems.
